Asking LEAD For Help

A LEAD participant shares how consistent case management, medical advocacy, and connections to treatment helped him reach sobriety, stay engaged in care, and move into stable, supportive housing.

A younger participant that has been with the program for almost two years has gone through detox many times but has continued to use after exiting programs. He has complex medical issues that required hospitalization and a mental health evaluation. His LEAD Case Manager met him at the hospital with his mother and advocated on his behalf immediately. He was able to get the evaluation and medical care he needed and was connected to additional resources. The Case Manager helped get the participant connected with a methadone clinic and the necessary aftercare to manage his medical needs. He has been consistent with his medical appointments, is currently in sobriety and connected with his mother in stable and supportive housing.
